Open Access available Monday-Friday from 8:00-4:00 with no appointment needed. Provides comprehensive mental health treatment providing support and assistance in navigating community resources, outpatient therapy, substance use treatment and support, psychiatry, etc.

Elevate Lebanon provides help with coordination and assistance in navigating the resources available in the Lebanon area. Housing and utility resources are available through our Connections department. Elevate Thrift supports the Rewards program that helps with costs for gas, replacement identification fees, and items of necessity. Elevate Work is the workforce development program that connects the unemployed and underserved population of our community with the necessary tools to achieve gainful employment.

Offers a variety of services including assistance with obtaining identification and securing employment/education, showers and laundry access are available, along with a variety of secondhand options for families in need.

Jordan Valley is open for walk-ins Monday-Friday from 7:40A-6:00P (opens at 7:00A on Mondays and Tuesdays) offering family medicine, pediatric care, primary and preventative care, immunizations, geriatric care, chronic disease management, addiction recovery, Hep C care, radiology, behavioral health, dental care, and oral surgery.

Lane Change is available Monday-Thursday from 9:00A-5:00P and Fridays from 9:00A-12:00P. Lane Change is a Christian organization committed to helping the community by providing hope for substance use through outreach, prevention, support, and education. Contact them by phone or at their physical address during business hours.
VA (Veterans Affairs):
- If care is already established within VA services, call Green Team at (573) 814-6486 to schedule an appointment
- If care is not previously established with VA services, call (573) 814-6000 and request verification of eligibility and outreach by a Veterans Justice Outreach Coordinator

Food pantry for families in need. Families can receive a one-time emergency box or can choose to register so they can receive food every two weeks.
Crisis Centers – Open 24/7 -- All locations can be reached at 1-833-356-2427 – listed are the two closest locations to Lebanon, MO – Call to inquire about transportation assistance/availability if needed
The goal of the Crisis Stabilization Centers (Crisis Access Points) is to provide a safe, welcoming, and less costly alternative to divert individuals away from jail and emergency rooms. Crisis Centers allow for immediate triage and stabilization, further observation, connecting the individual to ongoing care, and avoiding inpatient admission.

24-hour Crisis Lifeline accessible via online chat, text, or call providing support during a crisis, whether you or someone you know is experiencing a crisis, 988 is here to support you. 988 has the ability to dispatch a Crisis Response Clinician if necessary and if there is one available in the area.

Domestic and sexual violence shelter that aims to aid, shelter, and empower victims of domestic violence and break the cycle of abuse.

Local hospital to Lebanon offering general medical/surgical care, a 24-hour physician staffed Emergency Department, outpatient procedures, telemedicine for stroke and behavioral health, clinic specialty services, and virtual care and support provided to the inpatient area.
