Submit a Commendation or Complaint
Welcome to the Lebanon Police Department's submission page for commendations and complaints. Our goal is to strengthen community trust and enhance transparency within our department by providing a straightforward process for citizens to recognize exemplary officer behavior or report perceived police misconduct.
Purpose of the Program
“Our new complaint and commendations process is intended to strengthen community trust in the LPD,” said Police Chief Jerry Harrison. “This program allows us to recognize and celebrate exemplary behavior and outstanding service by our officers, while also providing a formal mechanism for reporting and addressing any perceived misconduct. We are committed to transparency and accountability, and we believe this initiative will help us build a stronger, more positive relationship with the community we serve.”
Submit a Commendation
If you would like to recognize a Lebanon Police Department officer for exemplary behavior or outstanding service, please use the following link to access the Commendation Form.
Complete the form online and email it to Angie Pollreisz at apollreisz@lebanonmo.org. Your commendation will be reviewed, and the officer will be recognized for their positive contributions.
File a Complaint
If you have a concern regarding perceived police misconduct, please use the following link to access the complaint Complaint Against Police Personnel Form.

Important Note: These forms are not intended for reporting crimes or criminal activity. For crime reports, please call (417) 532-3131 for non-emergencies or dial 9-1-1 in case of an emergency.
Additional Methods of Submission
- Over the phone: Call the Lebanon Police Department at (417) 532-3131.
- In person: Visit the Lebanon Police Department to submit your complaint.
- Anonymously: The LPD accepts third-party and anonymous complaints, provided sufficient information is included to facilitate an investigation.
Physical Availability of Forms
All LPD officers are required to provide a complaint form upon request to ensure accessibility for all community members. Forms are also available at the Lebanon Police Department.
